Silent Witness Form

Many people report incidents to faculty and administrators, believing that those officials will notify the police.  However, that is not necessarily the case.  Unless you have personally spoken with a police officer, you cannot be sure a report has been made.  When making a report, please provide as much of the following information as possible:

If the suspect's name is unknown give a description of the subject (i.e. clothing,appearance, or perhaps who the suspect was visiting on campus?)
