This form must be filled out online no later than Friday, February 29, 2008, and can be submitted by clicking on the "Submit"  button at the end of the document.

Additionally, please print a faculty recommendation form for each of your recommenders to include with their letters of reference.

Applicant should have taken at least one 200-level course in her research field. Please list relevant courses below along with the grades received.

Courses and Grades:


Two faculty recommendations are required with each HSRP proposal, one from the faculty sponsor and one from another faculty member. Recommendation forms should be signed by the student and given to each recommender. Recommendation forms should be returned to the student in sealed, signed envelopes for submission as part of the student’s proposal. Faculty may also send recommendation letters along with the signed forms directly to Julie Hemstreet, Honors Program, Sales Honors/Education Bldg.

Complete proposals, including faculty recommendation letters, are due by 12:00 noon on Friday, March 21, 2008, and should be delivered to Julie Hemstreet, Honors Program, Sales Honors/Education Bldg. All proposals will be reviewed by the Honors Committee.
