Required Medical Forms
There are two medical forms that are required for all new students. Students who do not submit these forms will not be allowed to attend class. Varsity Athletes and riders must also submit a completed Authorization of Use form. Please print and fill these forms out and retrun to the Student Health and Counseling Services office by June 30 for students entering in Fall 2013 or January 10 for students entering Spring 2014.
