Forms for New Students

Welcome to the first-year forms page. This section of the website contains all of the forms that you will need to complete prior to your arrival on campus. There are five categories of forms you will need to complete.
All forms are mandatory unless otherwise noted.  

The New Student Information Form is mandatory. This form is four pages and asks for basic information, residential (or non-residential) preferences, dietary likes and dislikes as well as general questions about your family. You will need to login with your username and password. Plan to complete all parts of the form at one time; this form should take about 20 minutes to complete.
 

There are several academic forms that students must complete prior to arriving for orientation.

The Academic Advising Questionnaire: Mandatory for all students and must be submitted by June 30th for students entering in the fall ’13 and January 10 for student entering in the spring ‘14. The information you include will be critical for your work with your faculty advisor and will be shared with only those individuals who can further assist in advising you. 

Online Placement Tests: Students who will be taking French, German, Spanish and Latin must take the online placement tests prior to August 1 for Fall 2013 or January 10 for Spring 2013. If you have never taken a language then you should not complete an online placement exam.

Plagiarism Test: In taking this mandatory plagiarism assessment, you will help us, help you to avoid the potential pitfalls of not correctly acknowledging another's thoughts, words or ideas. If you do not successfully pass this assessment, you will be required to attend a review session.

Quantitative Reasoning Test: Sweet Briar College's Quantitative Reasoning (QR) Test is a mandatory test taken by students in their first year and again in their senior year, for the sole purpose of assessing the QR component of SBC's General Education Program.

Entering Student Inventory (ESI): New students will receive instructions for completing the ESI via email in early June.

ePortfolio: Mandatory for all students and must be submitted by Aug. 1st for students entering in the fall of 2013, Jan. 10 for students entering in the spring of 2014. If you do not successfully submit this assignment, you will be required to attend a review session during Orientation. The Medical Form is mandatory for all students. The form is divided into two sections. You will need to complete the Confidential Report of Medical History. Your doctor will need to complete the Confidential Physical Form. If you are a rider and/or athlete then you must complete the additional Authorization To Use and Disclose Protected Health Information, Education Records and Other Information. Your medical forms must be completed prior to your arrival on campus. Students who do not submit medical forms will be unable to register for or attend classes.
Your photo ID.  You can use your webcam and take a picture for your campus ID - right on the webpage. You can keep taking a picture until you see one you like and then submit it. If you don't have a webcam, snap a photo that you really like of yourself and upload it. Make sure you submit a photo that adheres to the standards outlined in this section. You will recieve an email if your photo is unacceptable; please make sure you follow the guidelines provided. Your photo ID will be available when you arrive on campus.
Other forms. There are a number of other forms that are optional. Please make sure you check out the list and complete the form that are relevant to you.
